This page contains instructions of how to enroll as a patient of Dr. Thanon Family Medicine Practice.
Dr. Thanon is a female family physician. Until further notice, she currently works 2 days a week Mondays and Thursday 10am-4pm for appointments by phone. Starting August 11th additional day for in person visit at the clinic will be available on alternating Tuesdays and will need to be pre-arranged with Dr. Thanon in advance.
To accommodate your needs, in clinic days at Med Team Clinic will gradually be added as the province of Ontario and Ottawa Public Health continue to implement phase 3 and begin the transition to removal of the emergency state in the province recovery plan.

You will need to complete the fillable PDF enrollment form, which you can obtain by clicking on the PDF icon below. Type ( preferred ) or print in capital letters using a black or dark blue. Once this form is completed. Please save or scan/take a photo of it and email the clinic at:
Please also list your top 3 preferred dates for a meet and greet phone call appointment with Dr. Thanon. These dates must be within the next 30 days from your application date and falls within Dr Thanon availability days above. Please also specify if you prefer morning or afternoon appointments, or if it is doesn’t matter.
Please ensure to complete this form in its entirety as incomplete forms will not be accepted. We require you to provide both E-mail address and phone number when you complete the enrollment form, as these will be our means of communicating with you.
Once the clinic has verified that you have submitted a completed enrollment form. You will be sent an email with your confirmed meet and greet dates and time.
In preparation for your appointment, It is recommended to review Dr Thanon family practice clinic policies. Please read these policies. Take your time. The details are very important. Their purpose is to provide information about how the clinic operates so that it can meet the needs of its patients without misunderstandings or surprises. Please print out and sign the last page only and attach it with your replay email along with the completed enrollment form . Hard copy of any signed documents can be given to you upon your visit to the clinic.
