Med-Team Clinic Policies
1. Appointment & Scheduling Policy
• Patients must schedule appointments in advance via phone, online, or in person.
• Walk-ins are accepted based on availability, but priority is given to scheduled patients.
• Patients must arrive 15 minutes before their appointment to allow for check-in.
• Missed Appointments & Cancellations: Patients must provide at least 24 hours’ notice for cancellations.
Repeated no-shows may result in appointment restrictions or a fee.
2. Non-OHIP & Private Pay Services Policy
The following link will list specialties offered by the clinic, that may not be covered by OHIP and require private
payment.
Payment Policy for Non-OHIP Services:
• Patients must pay for non-OHIP services at the time of their visit.
• Accepted payment methods: Debit, Credit, Cash.
3. Billing & Health Card Policy
• Patients must present a valid Ontario Health Card (OHIP) at every visit.
• Expired or invalid health cards may result in the patient being billed directly for the visit.
• Patients without OHIP coverage must pay for their consultation upfront.
4. Privacy & Confidentiality Policy
• Med-Team Clinic follows PHIPA (Personal Health Information Protection Act) to ensure patient privacy.
• Patient medical records are securely stored and only accessible to authorized staff.
• Information will not be shared without patient consent, except as required by law.
5. Code of Conduct & Respect Policy
• Patients and staff must treat each other with respect and professionalism.
• Harassment, verbal abuse, or aggressive behaviour will not be tolerated and may result in refusal of service.
• Disruptive patients may be asked to leave and could face permanent discharge from the clinic.
6. Medical Records & Consent Policy
• Patients have the right to request access to their medical records by submitting a written request.
• A fee may apply for copies of medical records or transfer of records to another healthcare provider.
• Parents or legal guardians must provide consent for the treatment of minors under 16 years.
7. Emergency & After-Hours Care Policy
• Med-Team Clinic is NOT an emergency center. Patients experiencing an emergency must call 911 or go to the nearest hospital.
• For urgent but non-emergency concerns, patients can contact Telehealth Ontario (1-866-797-0000).
• After-hours medical inquiries will not be addressed until the next business day.
8. Infection Control & Clinic Safety Policy
• Patients experiencing fever, cough, or flu-like symptoms must inform the clinic before arrival.
• The clinic follows strict hygiene protocols to prevent infections.
• Masks may be required based on public health guidelines or individual doctor discretion.
9. Communication & Contact Policy
• Patients can contact the clinic via phone. (All voicemails will be addressed within 24 hours of receipt)*
• Medical advice will not be provided via email or phone; patients must schedule an appointment for medical concerns.
*No appointment cancellations can be made over a voicemail within 24 hours of the appointment
